Center for Hospice Care

Established in 1978, under the name Hospice of St. Joseph County, Center for Hospice Care (CHC) is an independent, community-based, not-for-profit organization that works to improve the quality of living through hospice, home health, grief counseling, and community education. With offices in South Bend, Plymouth, Elkhart and Mishawaka, Center for Hospice Care serves Elkhart, Fulton, Kosciusko, LaGrange, LaPorte, Marshall, St. Joseph and Starke counties in Indiana. Through December 2016, CHC has served more than 32,000 patients and is a premier, nationally recognized, award-winning agency. Caring exclusively for persons with life-limiting illnesses and their families/caregivers, CHC also operates two Medicare certified seven-bed inpatient units, known as Hospice House, at both its St. Joseph and Elkhart County locations. The agency also operates a freestanding community bereavement facility, Administrative and Foundation offices, and an outpatient Palliative Care Center, all located in Mishawaka. While many patients are covered by Medicare, Medicaid, private insurance plans, CHC provides its services to any person eligible, regardless of ability to pay. With a professional staff of 235 and more than 500 volunteers, CHC served 2,109 patients and their families in 2016 and continues to care for more patients than any other program in Indiana. By annualized numbers of patients served, CHC ranks in the top 3% of all hospice programs in the United States.
Center for Hospice Care (CHC) is preparing for the future by expanding our facilities and endowing many of our programs designed to help patients and grieving families. Cornerstones for Living: The Crossroads Campaign is focused on raising money to build a new 12-bed Hospice House along the St. Joseph River on our Mishawaka campus. We are also working to endow programs such as After Images—an art counseling program to help the grieving—as well as Camp Evergreen, a free camp for grieving children throughout the community. Funds raised during Give Local will go toward supporting these efforts.
